Personnel Forms

Upon hire, this form will provide the Office of Human Resources with an employee’s important personal information.
The Department of Homeland Security requires that all employees complete an I-9 employment-verification form before beginning any type of work. This form requires potential employees to present valid, original documents that verify identity and work authorization.  Acceptable forms of identification are listed on page 9 of the form.
Informs the Office of Human Resources with any departmental office changes such as staff title changes, transfers, pay changes, or classification updates.
Provides updates to personal information changes, such as permanent resident address or name, due to a move or change in marital status; this form may require supporting documentation to ensure proper processing.
This form provides instructions on how to update emergency contact information electronically through Self-Service Banner.
The purpose of this form is to request approval for filling a new or existing position within a department of the University.

Payroll Forms

The pay schedule will provide bi-weekly and student employees with pay period ranges, time sheet submission deadlines and pay dates.
Direct deposit is mandatory for all Ohio Northern University employees. A voided check or a letter from the employee’s banking institution providing the routing and account numbers needs to accompany the direct deposit form.
This form specifies the federal tax withholdings to be deducted from an employee’s paycheck. All employees are subject to federal withholdings.

If your permanent address is in the State of Ohio or not in the states of KY, MI, PA, WV, or IN, please complete this form. This form identifies the state tax withholdings to be deducted from an employee’s paycheck.   The state of Ohio taxes all individual whose work is conducted in Ohio.

If your permanent address is in a neighboring state (KY, MI, PA, WV or IN), please complete this state tax form to waive Ohio State Tax.

Elect a tax-deferred voluntary contribution into your retirement account to be deducted from your paycheck.  This election can be an on-going reduction or a one-time salary deduction.  This form must be filled out anytime a change to the amount or frequency is requested.
Deposits a pre-tax amount into employee’s Health Savings Account at a financial institution of their choosing.  A voided check or a letter from the employee’s banking institution providing the routing and account numbers for the Health Savings Account needs to accompany this form.
Complete this form to request payment for a pre-authorized grant; this form requires supporting grant agreement documentation to ensure proper processing.
Informs the Office of Human Resources of any bereavement leave taken by an employee, this is for payroll leave documentation verification purposes.  Guidelines for bereavement leave can be found in the staff handbooks located on the Policies, Procedures & Compensation webpage. 
Gain access to their W-2 form quickly without waiting for a paper form to be mailed to their home.  If this request is made, your W-2 will be available in Self-Service Banner so long as you have access to ONU email.
Provides employee’s with instructions regarding their W-2 tax return forms.
Complete this form to request extra earnings funding and payment; supporting documentation may be requested with submission of this form.
The purpose of this form is to change or add an employee as an approver for timesheet or leave approving purposes.

Upon summer camp hire, this form will provide the Office of Human Resources with an employee’s important personal information.
Requires an employee to acknowledge and accept their responsibilities as an employee who has access to financial or personnel information as to protect the employees’ right to privacy.  This statement must be acknowledged and signed.
Identifies the computer and internet responsibilities expected of University employees.  This statement must be acknowledged and signed.
This form authorizes an employee to operate a motor vehicle insured by the University.  Please visit Safety and Security to complete this form.
