Address/Personal information changes may be initiated utilizing one of the following:
- Change of Address and Personal Information - PDF
This pdf form may be filled out, printed, SIGNED and mailed to or taken to the Registrar's Office. DO NOT SUBMIT THIS FORM VIA E-MAIL.
- Via Self-Service Banner (the ONU Portal) by selecting: Personal Information, then Update Address(es) and Phone(s)
- Via email from your ONU email account only to registrar@onu.edu.
