Student Request Procedure - Signed Form

Enrollment Verification for purposes other than loans (i.e. car insurance, health/medical insurance, jury duty, tax purposes, etc.) may be obtained either by a request via the ONU Web portal (Self-Service Banner) or by completing and signing an Enrollment Verification Request form and delivering it to the Registrar's Office. 

Please allow a minimum of two working days for processing Enrollment Verifications and five for mailed forms to be received.
E-mail requests are not acceptable as federal law, FERPA, requires the student's written signature to release any document containing the student's SSN.
Enrollment verifications issued before a term has begun will clearly state that the student has registered for a term that has not begun.

Former students may use the form as well by including your dates of attendance at ONU in the "other" space near the bottom of the form and provide your date of birth in lieu of an ONU ID number.  According to FERPA rules, all forms must be SIGNED by the student or former student.  

Signed form should then be submitted to:

or by fax to 419 772-2627
or as an email attachment to registrar@onu.edu.
After faxing or emailing as an attachment you may follow up with a phone call to the Registrar's Office to verify receipt if you wish 419 772-2024.  
The Registrar's Office mails or faxes the generated enrollment verifications to the designated party but does not email them out.
